Botox is one brand of botulinum toxin type A, a purified protein that blocks the signal from nerve to muscle where it is injected. Tiny doses soften the movement that folds the skin into expression lines. It does not fill, lift or tighten skin, and it does little for lines that are there because of lost volume or sun damage rather than movement. The effect wears off on its own as the nerve endings recover.
Several brands do the same job. They differ in formulation, and their units are product specific: 20 units of one brand is not the same dose as 20 units of another[1]. A good practitioner will tell you which product they use and why.
The US label for Botox Cosmetic sets these doses[1]. Practitioners adjust for your muscles, and many people need less, particularly for a softer result. Men often need more.
| Area treated | Botox Cosmetic label dose | Injection points |
|---|---|---|
| Frown lines between the brows | 20 units | 5 |
| Crow's feet | 24 units (12 a side) | 6 |
| Forehead lines | 20 units, given with the frown lines, so 40 in total | 5 plus 5 |
| All three areas together | 64 units | 16 |
Clinics price by area, by unit or by package. A quote for "one area" tells you little until you know how many units it includes, so ask. Two clinics charging the same per area can be giving you very different doses.
It suits people who see lines when they frown, raise their brows or smile, and who want a subtle result that wears off if they do not like it. Treating lines that only show on movement usually gives a more natural result than chasing deep creases that are etched in at rest.
It is not for you, or needs a doctor's view first, if any of these apply[1]:
If what bothers you is hollowness, sagging or skin texture, Botox is the wrong tool and an honest practitioner will say so.
Most side effects are minor and short lived: pinprick bruises, redness, tenderness and a headache on the day. The ones people worry about are a drooping eyelid or brow, which happen when the product affects a neighbouring muscle. They fade as the product wears off, usually over weeks. These are the rates in the label's clinical trials[1]:
| Area | Most common side effect in the label's trials | How often |
|---|---|---|
| Frown lines | Drooping eyelid | About 3 in 100 |
| Crow's feet | Eyelid swelling | About 1 in 100 |
| Forehead lines | Headache | About 9 in 100 |
| Forehead lines | Drooping brow | About 2 in 100 |
All botulinum toxin products carry a US boxed warning that the effect can spread beyond the injection site. The label notes no definitive serious cases of this at the approved cosmetic doses[1]. Get urgent medical help if you have any trouble swallowing, speaking or breathing after an injection.
The serious harm in recent years has not come from licensed product in a clinic. It has come from fake or unlicensed toxin, often injected at home, at parties or in salons by people with no medical training.
How to protect yourself:
The NHS puts the cost in the UK at about £100 to £350 a treatment, depending on the clinic and the area[2]. Cosmetic Botox is not available on the NHS. In the US, plastic surgeons reported an average fee of $435 for neuromodulator injections in 2023[3]; medspa and big city prices vary on either side.
Because it wears off, think in yearly terms. Three treatments a year at the NHS range comes to roughly £300 to £1,050 in the UK, or about $1,300 a year at the US average.
What pushes the price up: a doctor or senior nurse prescriber injecting, a prime city location, more areas and more units. A price far below the local going rate is a reason to ask what product is being used and who is injecting, not a bargain.
